Minor Policy

At Sport Performance Therapy, we prioritize the well-being and safety of our young clients. Before commencing any treatment for a minor, we require a formal consent process to ensure that all parties involved are informed and comfortable with the provided services. The following policies outline the necessary steps for the treatment of minors:

Minor Consent Form: Prior to the initiation of any treatment for a minor, a parent or legal guardian must sign the Minor Consent Form. This form ensures that we have the necessary information and permissions to provide the best care for your child.

Mandatory Supervision: For the entire duration of the minor’s treatment(s), it is imperative that a parent, legal guardian, or an appointed adult representative remains present in the treatment room.

Open Communication: We encourage open and transparent communication between our therapists and the supervising adult. Any concerns, changes in the minor’s health status, or additional information that may impact the treatment should be promptly shared.
